Referrals

You can complete our online referral form with some basic information about the person and the support they are looking for.

A participant can refer themselves, or a referral can be made by a family member, loved one, Support Coordinator, Plan Manager, hospital, allied health professional or another person involved in the participant’s support.

Our referral form asks for basic information about the person making the referral, the type of support they’re looking for and when they would like support to start.

No. You can tell us what you’re looking for and we’ll discuss the next steps with you.

Our team will review the information provided and contact you to discuss the referral and next steps.

We aim to respond to enquiries and referrals within one business day.

UnitedTribe plans to expand its services as the organisation grows. Future services may include areas such as Supported Independent Living, Short Term Respite, Group Support, nursing supports, complex care, support coordination and allied health partnerships.

These services will only be presented as available once UnitedTribe is ready and appropriately registered to provide them.
